A second version used a Likert scale for both the 7 competencies and 24 sub-competencies.The Gap‰ÛÒKCS Assessment Form can be used for self-assessment; an adapted version can be used by actual or simulated patient/family members. It contains the original seven competencies and two additional dimensions: demonstrates empathy and communicates accurate information.
