SUMMARY:tEACH webinar\, “If you were to get worse…” Goals of Care Con
 versations in Acute HealthCare Settings
DESCRIPTION:Speakers:\n\nMandy Williams and Kim Taylor\n\nAlmost one third 
 of patients admitted to hospital for acute care are likely to die within t
 he next 12 months. It is important that as health professionals we are abl
 e to identify patients who are approaching death due to progressive illnes
 s or frailty on admission to hospital. This will enable us to initiate con
 versations soon after admission about their condition and discuss care opt
 ions that reflect patient’s goals\, values and priorities.\n\nHolding sk
 illful patient-centred conversations with patients approaching the end of 
 life can have a profound impact on patients’ quality of life and experie
 nce. Evidence shows that these ‘goals of care’ conversations can:\n\n 
 	 Reduce unwanted escalations of care at the end of a patient's life\n 	
  Improve clinicians’ documentation and delivery of patients’ preferen
 ces\, including their preferred place of care at the end of their life\n 	
  Reduce emergency hospital admissions and make good use of NHS resources\
 n\nIn this workshop Mandy &amp\; Kim share content and experiences from th
 e newly launched 2 day course designed specifically for mid-grade doctors\
 , nurses and AHP’s who have these challenging conversations with patient
 s in acute care settings. The workshop asks you to consider how do we iden
 tify these patients\, what is currently happening in your service areas an
 d what support/training is provided for healthcare professionals to have t
 hese conversations? You will experience some of the activities and resourc
 es from the course and there will be opportunity for questions\, discussio
 n and sharing of ideas.\n\nMandy works as a Senior Tutor in Clinical Commu
 nication Skills on the undergraduate medical course at Cambridge Universit
 y Hospitals Trust and as an independent coach and trainer on postgraduate 
 communication skills courses.\n\nKim is a simulated patient and simulated 
 patient trainer with over twenty years involvement in medical education. K
 im has extensive experience in breaking bad news and end of life care trai
 ning with healthcare professionals across specialties and disciplines.\n\n
